Direct Primary Care is a new type of healthcare delivery system which involves a direct payment between the healthcare provider and the patient in return for a “basket” of services. Those services in the Maple Medical™ Be Well Network include physicals, follow-ups, basic imaging studies, basic lab studies, as well as cardiac echo, stress testing, ultrasound and bone density. In New York State, we are required to charge a $35 fee for a sick visit.
Direct Primary Care is an affordable alternative in today’s tumultuous times, which will provide you with the opportunity to get most of your primary care needs serviced in one practice. This includes chronic care management for hypertension, diabetes, high cholesterol, heart disease and lung disease, all of which contribute significantly to morbidity and mortality in healthcare. Our emphasis is on early detection and treatment of disease.

1. What is Be Well?
Be Well is a direct primary care network. Direct primary care is a new way to get high quality, competitive healthcare from a doctor for a reasonable monthly fee.
2. What does Maple MedicalTM Be Well cover?
Many of your primary care needs, including physicals, follow-ups, echos, basic x-rays, stress tests, ultrasounds and basic lab tests.
3. Can Be Well take care of chronic conditions?
Chronic conditions like asthma, high blood pressure, diabetes, high cholesterol and thyroid disorders are covered.
4. What specialty services are covered?
Specialty services covered are those oﬀered within Maple Medical, such as endocrinology, cardiology, pulmonary, neurology and rheumatology.
5. How much does Maple Medical™ Be Well cost?
$65 a month.
6. What about sick visits?
Under New York State law, we are required to charge $35/sick visit.
7. Is Be Well insurance?
No. Be Well is not and does not replace an insurance policy.
8. Is catastrophic illness or hospitalization covered?
No. Only basic primary care is covered.
9. Can I get out of the annual contract?
10. Are gynecology and pediatrics covered?
Not at this time.
11. Is direct primary care the same as concierge medicine?
No, there is a significant diﬀerence. Concierge medicine is a diﬀerent price point usually starting at $100 per month and upward. Many concierge practices also bill insurance companies.
